From 27 April 2026 it exists only in electronic form in the MOS system: the employee invites the firm by e-mail, and the firm completes the form and signs it via login.gov.pl. Whether the voivode even takes up the case, and for how long the permit is issued, depends on the agreement of this document with the contract.

What Annex no. 1 is for

The voivode does not assess the contract itself in isolation from the rest of the file. For the voivode, Annex no. 1 is a statement of the entity entrusting work: who employs, in what position, for what pay, in what working time and under what type of contract. According to the information on the MOS portal, on this basis the office checks three things: whether the purpose of residence exceeds 3 months, whether income from work is adequate, and whether the foreigner will have health insurance (for an employment contract (umowa o pracę) and a mandate contract (umowa zlecenie)).

The conditions entered in the annex then end up in the decision. Under Article 118 of the Foreigners Act, the decision names the entity entrusting work, the position or type of work, the lowest remuneration, the working-time dimension and the type of contract. Any later discrepancy between these data and reality may mean a need to change the permit.

How the document flow in MOS looks in 2026

The foreigner opens a MOS account and fills in their own application, giving the employer's e-mail address in it.

After clicking the option to send the form to the employer, the system sends a link to the electronic Annex no. 1 to that address. The MOS portal expressly recommends that the foreigner make sure the employer can sign the annex via login.gov.pl.
